Payment for Therapy Services

I participate in Blue Cross Blue Shield, United healthcare, EHP, JH Advantage and Medicare and do all the billing for you. I bill all other insurance companies and will work with you to negotiate a rate that is fair. I accept credit cards with a 4% fee, cash or check. You are responsible for all fees not paid for by insurance that have not been negotiated.

 SAP services including SAP Assessments, DOT Physical Assessments and MDOT assessments are payable by cash or credit card. The range of fees charged is $150 - $500.
